Chronic Conditions
Chronic conditions (Asthma,Allergies,Diabetes, Seizure)
Families are required to submit:
- physical examination forms
- updated action plans and
- medications promptly at the start of each school year.
- Action plans (Asthma, Allergy, seizure and Diabetic 504 (if applicable).
- Immunization records
- Forms can be picked up at the school's Health office.
